The relationships of zinc protoporphyrin by hematocrit with blood lead and other lead exposure indices



Abstract
The measurement of zinc protoporphyrin(ZPP) in the whole blood with potable hematofluorometer is often used as primary screening tool to identify high lead absorption in the field setting for its' convenience and direct readability. The current
unit of
ZPP with hematofluorometer adopted in Korea is ug per 100 ml of whole blood with the adjustment of hematocrit(Hct) correction of 42% which is the unit of OSHA in U.S.A. But the average of Hct of male and female is quite different and real
expected
ZPP
can be under or over estimated by individual Hct value if it is adjusted with Hct 42%. This study intended to investigate the effect of Hct adjustment of ZPP on the inter-relationship of other lead exposure indices such as lead in whole
blood(PbB)
and
delta-aminolevulinic acid(DALA) and to check whether hematocrit adjusted ZPP will change the validity of ZPP in screening of lead workers under current lead regulation of Industrial Safety and Health Act. Authors studied 435 male and 123 female
lead
workers working in storage acid battery and secondary smelting industries. The selected variables for this study were PbB, Hct and hemoglobin(Hb) in the blood and DALA in urine.

@EN 1. While the mean values of Hct in male and female workers were 46.5% and 40.2%, those of Hb in male and female workers were 15.7%g/dl and 13.2%g/dl, respectively.
2. While the correlation coefficients between PbB and un-corrected ZPP were 0.604 in male lead workers and 0.635 in female lead workers, those between PbB and corrected ZPP were reduced to 0.586 in male and 0.603 in female lead workers, but the
